A Retrospective Analysis of Tumor Volumetric Responses to Five-Fraction Stereotactic Radiotherapy for Paragangliomas of the Head and Neck (Glomus Tumors)

Abstract: Background: Skull base paragangliomas (SBP) are locally expansile tumors that can be treated with stereotactic radiotherapy with favorable results. This report describes the results of 31 patients with SBP treated with CyberKnife radiotherapy delivering a total dose of 25 Gray in five fractions. Methods: All patients treated with five-fraction CyberKnife radiotherapy at a single institution were identified between 2007 and 2013. Tumor volumetric analyses were performed to assess responses to radiotherapy. Resu… Show more

“…[19][20][21] Patients in our study experienced good LC, 90% were locally controlled, which is slightly lower than in similar published studies. 3,8,9,11,12,14,15,[22][23][24][25] The group of patients that had local progression were all treated with conventional radiotherapy techniques, using a limited number of treatment fields (1)(2)(3)(4). Interestingly 4 of 5 patients with extended follow-up (> 239 months) experienced progressions either locally or at other sites.…”

“…However, late radiation toxicity needs to be considered. 3 Recently, stereotactic RT has become more available [10][11][12] and the results of single institution series or meta-analyses indicate a potential for higher rates of tumor control 13,14 with low morbidity in comparison to older studies using conventional RT. 15 We present an analysis of outcome data and toxicity data of HNPGL patients treated with RT in different institutions from the Rare Cancer Network (RCN) (http://www.…”

“…Even so, the risk of malignant transformation is low and previous literature has shown that radiotherapy at the skull base can be administered safely . Courses of 45 Gy delivered over 25 fractions as well as 25 Gy delivered over 5 fractions have been shown to be safe and efficacious for skull base paragangliomas and may be a suitable preliminary treatment plan for patients undergoing radiotherapy …”

“…4,5 Therefore, management options include observation and primary radiotherapy, which can be used to achieve local control, especially in the elderly. [1][2][3][4][5][6] However, advances in skull base microsurgical techniques have made complete excision with acceptable functional outcomes possible. [1][2][3][4][5] Paragangliomas exhibit a well-known phenomenon of tumor angiogenesis and secrete certain factors such as VEGF, PD-ECGF, and endothelin-1.…”
