A Retrospective Study Comparing the Different Surgical Procedures for the Treatment of Hydatid Disease of the Liver

Abstract: Background/Aims: The purpose of this study was to compare the security and effectiveness of the different surgical procedures used for treatment of the liver infestations with larvae of the dog tapeworm (Echinoocccus granulosus). Methods: 55 patients with echinococcal cysts in the liver were treated surgically at our department between 1987 and 1999. The patients were grouped according to the mode of surgical technique and comprised 16 anatomical liver resections, 26 pericystectomies and 13 endocystectomies. T… Show more

“…Many authors in the recent literature agree that radical resection of the hydatid cyst should be preferred when feasible [14–17], although some believe complete resection does not correlate with superior results [18, 19]. In accordance with the mentioned reports we did not find a statistically significant difference in cyst size necessitating the two operations nor have we found a difference in severe complication rate, blood transfusions required in the operation room, or length of hospital stay.…”

“…The surgical approach remains the cornerstone of the treatment for hydatid disease [ 1 , 3 , 7 , 10 , 15 , 31 , 32 , 35 ]. Some suggest that a combination of surgery and antiparasitic agents is the preferred treatment as it is associated with low morbidity, mortality, and recurrences rates [ 10 , 36 ]. Surgery of liver hydatid cysts is intended to remove the parasite completely by a procedure that includes safe, complete exposure of the cyst, safe decompression of the cyst, safe evacuation of the cyst contents, sterilization of the cyst, treatment of biliary complications, and management of the remaining cyst cavity.…”

“…Radical operations, such as liver lobectomies and atypical resections, have also been proposed [6,20,24]. Even though they achieve better results in relation to the recurrence of the disease [18][19][20], conservative surgical management is still preferred by many centres with wide experience, because of the lower morbidity and mortality rates [2,7,22]. Omentoplasty has been widely recommended as the procedure of choice [7,9,10,15], particularly in the presence of an infected liver hydatid cyst [3].…”
