A Retrospective Study of Buprenorphine and Norbuprenorphine in Human Hair After Multiple Doses

Abstract: The analysis of hair has been proposed as a tool for monitoring drug-treatment compliance. This study was performed to determine if buprenorphine (BPR) and norbuprenorphine (NBPR) could be detected in human hair after controlled administration of drug and to determine if segmental analysis of hair was an accurate record of the dosing history. Subjects with dark hair (six males, six females) received 8 mg sublingual BPR for a maximum of 180 days. Single hair collections were made once after BPR treatment and st… Show more

“…Additionally, in the present study, there was no statistically significant difference between ratios of buprenorphine:norbuprenorphine in washed compared to unwashed hair. Furthermore, Kintz (11 ) and Tracqui et al (13 ) used a methylene chloride wash before analysis, as was done in this inves- (14 ) and the current protocol differed from other studies by including dissolution of hair with strong base, and extraction of buprenorphine and norbuprenorphine at pH 10.5. This pH was selected to enhance recovery of norbuprenorphine.…”

“…Wilkins et al (14 ) reported buprenorphine hair concentrations for 12 participants receiving 8 mg of buprenorphine daily for up to 180 days. Buprenorphine or norbuprenorphine was detected in 11 of 12 individuals.…”

“…No derivatization is required and glucuronide metabolites can be quantified without the need for costly and time-consuming hydrolysis. LCMS and LCMSMS have been applied for BUP and metabolites’ analysis in a wide variety of matrices including, urine [1,1928], hair [21,23,25,2932], sweat [33], meconium [34], breast milk [35], placenta [36], and umbilical cord [37]. In plasma, several methods were published for BUP alone [18,38], BUP and NBUP [23,3941]; BUP, NBUP, and glucuronides after hydrolysis [25,42]; BUP, NBUP and BUPG [43]; and all four analytes [1,15,44].…”

“…Enfin, les cheveux ont également été testés [29], avec un seuil de positivité fixé à 10 pg/mg sur une prise d'essai de 30 mg de cheveux, permettant d'obtenir 8 % de faux positifs et là encore l'absence de faux négatif. Contrairement à la méthadone, les concentrations les plus importantes retrouvées dans les cheveux sont celles du métabolite et non celles mesurées pour le produit parent [35,36].…”
