A Review of Patient Safety Measures Based on Routinely Collected Hospital Data

Abstract: The literature on patient safety measures derived from routinely collected hospital data was reviewed to inform indicator development. MEDLINE and Embase databases and Web sites were searched. Of 1738 citations, 124 studies describing the application, evaluation, or validation of hospital-based medical error or complication of care measures were reviewed. Studies were frequently conducted in the United States (n = 88) between 2005 and 2009 (n = 77) using Agency for Healthcare Research and Quality patient safet… Show more

“…[1][2][3] Primary care physicians perceive time limitations and disorganized work systems as factors that influence safety, 4,5 with communication, medication management, and test results tracking believed to be common sources of avoidable error. 2,3,6,7 Work is underway to develop consensus about key processes and metrics to promote systematic safety monitoring and reduce risk in primary care. 8 While working to develop these metrics, culture assessments have been used as indirect safety measures.…”

“…Over the past two decades healthcare globally has awoken to the concept of patient safety both as a critical component of care quality that is to be strived for and as an outcome measure to be monitored and audited [1,2]. This awakening [3,4] and concurrent shift in policy agenda [5,6] have triggered a burgeoning science of clinical patient safety.…”
