A Review of Selective Salpingography and Fallopian Tube Catheterization

Abstract: Use of selective salpingography and fallopian tube recanalization has revolutionized the diagnosis and treatment of infertility. Selective salpingography, a diagnostic procedure in which the fallopian tube is directly opacified through a catheter placed in the tubal ostium, has been used since the late 1980s to differentiate spasm from true obstruction and to clarify discrepant findings from other tests. In fallopian tube recanalization, a catheter and guide wire system is used to clear proximal tubal obstruct… Show more

“…On the other hand, failed fertilization and failed implantation were found to be the main causes of failed IVF used for managing these cases and the main causes of unexplained infertility in these cases also. The clinical use, advantages and disadvantage of the previous techniques used for diagnosis of ovum picking were identified by other authors [7,8]. But most of these studies which included the identification of these diagnostic techniques, if not all of them were used essentially for determining the tubal patency.…”

“…As described earlier, a falsely positive finding of occlusion of the fallopian tubes may result from either transient uterine contractions or other technical factors. Selective fluoroscopic salpingography or hysteroscopically directed tubal catheterization improves diagnostic accuracy and can differentiate true from false tubal blockage related to spasm, intraluminal debris, or minimal scarring (24)(25)(26); tubal patency can be achieved in over 80% of cases. When tubal disease is more severe, e.g., salpingitis isthimica nodosa, fluoroscopic or hysteroscopic recanalization is more challenging technically and less often successful (25,26), and IVF generally is the better treatment option.…”

“…A review [8] evaluating results with this technique in 1,466 patients reports a successful recanalization of the proximal fallopian tube in 71-92% of recanalization attempted. Pregnancy rates after the procedure have been variable among series, with an average rate of 30% during follow-up.…”
