A review on thromboangiitis obliterans pathophysiology: thrombosis and angiitis, which is to blame?

Abstract: A century has passed since thromboangiitis obliterans (TAO), or Buerger's disease, was first described, but the etiology remains unclear. It is still uncertain as to whether thrombosis or vascular inflammation is the first event. TAO is an episodic inflammatory and thrombotic-occlusive vascular disease of unknown origin. The involvement of the distal vessels and nerves within the neuro-vascular bundles occurs almost always in legs and occasionally in arms. The cumulative data demonstrate that at the cellular a… Show more

“…It affects both small and medium-sized vessels of the extremities. The thrombotic and inflammatory changes associated with TO cause vascular changes with associated arteriographic findings[9-11]. Hypothenar hammer syndrome, a post-traumatic pathology arising from repetitive hitting of the hypothenar region of the hand, can cause ulnar artery occlusive disease with thromboembolism and resulting ischemia to the digits[12,13].…”

“…Thromboangiitis obliterans (TAO) is an episodic, sharply segmental, inflammatory and thrombotic‐occlusive peripheral vascular disease with unknown etiology that can lead to gangrene and tissue loss . It particularly affects young males from low socioeconomic situations and its progression and outcome have a close relationship with tobacco consumption .…”

“…2 Patients with BD often suffer from a circadian rhythmic burning pain which usually starts before the onset of ischemia. 7 Rickettsia can activate the kinin-kallekirin system, which plays a role in tingling and burning pain sensations. 8 Recently, the activation of kinin system in BD has been reported.…”

“…13 This could be the reason why pain in BD patients responds well to sympathectomy. 7 Endothelial cell infection with Rickettsia may explain the involvement of small-and medium-sized arteries, as the slow velocity of blood flow in these vessels would allow sufficient time for attachment of Rickettsia to endothelial cell membranes. 2 An inflammatory reaction may also occur around the infected cells.…”
