A Risk-Benefit Assessment of Drugs Used in the Eradication of Helicobacter pylori Infection

Abstract: Helicobacter pylori is the cause of chronic active gastritis and predisposes to peptic ulcer disease (PUD). Furthermore, H. pylori is linked to the pathogenesis of gastric lymphoma and gastric cancer. However, treatment of this infection has proven difficult. In the last decade, many antimicrobial compounds have been studied extensively as monotherapy as well as in combination with bismuth or acid-suppressive drugs. The individual drugs and the most important eradication regimens are discussed with special reg… Show more

“…Historically, the first substances in use were bismuth salts (bismuth subcitrate and bismuth sub salicylate) [66], followed by several antibiotics among the 5-nitroimidazoles, penicillins and chinolones. Antibacterial monotherapy was ineffective in the eradication of H. pylori, since such regimes typically achieved cure rates of < 20 % after 2 to 4 weeks of therapy [67]. Use of mono therapy with antibiotics was eventually strongly discouraged due to the possible risk of development of antibacterial resistance [66].…”

“…Diese divergierenden Ergebnisse beruhen zum gröûten Teil darauf, dass sehr heterogene Regime bis Mitte der 90er Jahre zur Anwendung kamen, allerdings spielten auch die Güte der Erfolgsevaluation und z. B. die Resistenzlage unabhängig von anderen populationsbedingten Faktoren und Nebenwirkungen eine Rolle [5].…”
