A Scheme for the Ante‐Natal Prediction of ABO Haemolytic Disease of the Newborn

Abstract: Summary An investigation of 8,000 ante‐natal patients showed that 260 had a positive “modified” indirect anti‐human globulin test against A1 and/or B human red cells. In 116 cases the patient was ABO incompatible with her husband. A diagnosis of ABO haemolytic disease of the newborn was made on 24 babies born to these 116 patients. Three babies were exchange transfused. Using the information gained from this investigation we propose and describe a scheme suitable for the routine ante‐natal prediction of ABO ha… Show more

“…In contrast, in a previous investigation of a larger material an increasing frequency of haemolysins with increasing number of pregnancies has been found [Ames and Lloyd, 1964], This discrepancy is most likely due to the relative smallness of the present series. There was no correlation between C3 phenotypes and the number of pregnancies in the present material, however, which could cause a spurious correlation between C3S and antibody titre.…”

“…Many attempts have been made to achieve this aim by use of the titre of 'non-inhibitable' antibodies based on the work of Witebsky [65] and serum [65], accaicia [28] dextrans [36] as well an antiglobulin reagents [1,13,14,15,68] have all been used to demonstrate the 'non-inhibitable' antibodies following partial neutralisation of the sera with soluble blood group A/B substances.…”

“…occur in approximately 25% of all group O maternal sera ( [13] and table VI) the incidence of ABO HD is rela tively low, [1,12,34,36,42,43,51,53,54] occurring in only about 0.8% of all the infants of group O mothers.…”

“…25 Severe ABO HDFN which required exchange transfusion was relatively rare in few analyses; none among 1500 neonates in one study, 3 out of 14,000 consecutive births in another study, 3 out of 8000 births in a third study and 6 out of 5704 infants in fourth one. 26,27,28 Falterman C G and Richardson J has observed that transfusion of adult A or B red cells with more antigenic sites might undergo rapid destruction which could result in severe jaundice, kernicterus, haemoglobinuria and even death. 29 In a Saudi Arabian research, the value of phototherapy along with IVIG was compared with phototherapy alone in reducing the requirement for exchange transfusion in ABO HDFN.…”
