A Serotype VIII Strain among Colonizing Group B Streptococcal Isolates in Boston, Massachusetts

Abstract: Maternal colonization with group B Streptococcus (GBS) is a risk factor for neonatal GBS disease. Whereas serotypes Ia, Ib, II, III, and V are prevalent in the United States, types VI and VIII predominate in Japan. Recently, a serotype VIII strain was detected among 114 clinical GBS isolates from a Boston, Mass., hospital.

“…Disc diffusion testing was also performed with erythromycin (15 mg) and clindamycin (2 mg) discs, following NCCLS guidelines. Capsule serotyping was performed using serological methods and standard reference antisera (Paoletti et al, 1999). Typing for the major GBS surface C proteins was performed using PCR methods and primers designed to detect unique regions of the genes for the alpha C protein, Alp1, Alp2, Alp3, Rib and beta C protein (L. C. Madoff, unpublished results).…”

“…In Denmark, seven instances of invasive disease isolates belonging to CPS VIII among nonpregnant adults were reported from 1999 to 2002 (51). Also, one isolate colonizing a pregnant woman in Maryland was recognized as CPS VIII (52). Lately, the frequency of CPS VIII is generally low outside Japan, and studies often describe few isolates (53,54).…”

“…One study assessed a collection of GBS isolates classified as NT by serological methods and went on to characterize them by a number of different techniques (immunodiffusion, pulsed-field gel electrophoresis [PFGE], and multilocus sequence typing) (59). This resulted in all isolates being serotyped and found to represent all types, excluding IV and VII to IX, with instances of multiple serotypes per isolate, which has been described previously (52). This may represent an incidence of capsular switching, a phenomenon that involves the genetic exchange of capsular genes and thus enables expression of alternative capsule types, and it is proposed that such horizontal transfer may be responsible for the capsular locus diversity (60)(61)(62).…”

“…Studies from neighboring regions, such as the United Arab Emirates, revealed a predominance of serotype IV followed by types Ia, III and V (17), while Kuwait had similar serotype distribution to our population (serotype III: 24.3%, serotype V: 21.8%, serotype Ia: 12.9%, serotypes II and VI: 7.8% each) (18). Serotype distribution in other countries as the United States showed the prevalence of five serotypes including Ia, Ib, II, III and V. Lately, predominance of type VIII was noted in Boston, USA (19).…”
