Patients with implantable left ventricular assist devices (LVAD) are at risk of ventricular arrhythmias but these may be hemodynamically tolerated. An electrocardiogram (ECG) is essential to determine whether an LVAD-supported patient is experiencing a ventricular arrhythmia. Access to 12 lead ECG is predominantly in healthcare facilities. Implantable LVAD also cause significant electromagnetic interference leading to artefacts on ECG. We report a patient on Heartmate 3 LVAD with a diagnostic quality 6 lead ECG obtained with an AliveCor device during an episode of sustained palpitations. The AliveCor device may be used for remote identification of ventricular arrhythmias in LVAD patients.

What training should psychiatrists have to interpret six- and 12-lead electrocardiograms?

To monitor for drug-related cardiac arrhythmias, psychiatrists regularly perform and interpret 12-lead (12L) and, increasingly often, six-lead (6L) electrocardiograms (ECGs). It is not known how training on this complex skill is updated or how well psychiatrists can interpret relevant arrhythmias on either device. We conducted an online survey and ECG interpretation test of cardiac rhythms relevant to psychiatrists. A total of 183 prescribers took part; 75% did not regularly update their ECG interpretation skills, and only 22% felt confident in interpreting ECGs. Most participants were able to recognise normal ECGs. For both 6L and 12L ECGs, the majority of participants were able to recognise abnormal ECGs, but fewer than 50% were able to correctly identify relevant arrhythmias (complete heart block and long QTc). A small number prescribed in the presence of potentially fatal arrhythmias. These findings suggest a need for mandatory ECG interpretation training to improve safe prescribing practice.

Cardiovascular diseases are the main cause of death across the globe. Therefore, it is critical to detect them at an early stage, while an electrocardiogram (ECG) presents a widely used tool for making diagnosis. There is a large number of ECG devices on the market, with different features that suit a wide range of users, but simultaneously it poses a problem for the end user because they have a difficulty in selecting the appropriate device for their exact needs. Current trends in the usage of contemporary mobile ECG devices are identified and analyzed. In this study authors reviewed numerous mobile ECG devices with either FDA approval or CE mark that are currently available on the market. All listed devices are presented with all technical features that could be found. Furthermore, an interactive sunburst diagram for the selection of mobile ECG devices is created, and published, to help everyone easily make a selection.
