Combinations of extracorporeal shockwave lithotripsy (ESWL) and endourological and open surgery have been used for management of forgotten double-J (DJ) stents; however, there are only a few case reports or case series in literature. We present our experience of managing 28 cases of forgotten ureteric stents of whom three patients died because of complications after intervention. We retrospectively reviewed the hospital records of 28 cases of forgotten DJ stents from 2000 to 2013. The details reviewed included indications for stent placement, indwelling time, presenting complaints, laboratory, radiographic, nuclear scan findings, management techniques, and complications. Extensive review of literature was done. Mean patient age was 37.7±14 years. Mean indwelling time was 102.9 months. The commonest presenting complaints were irritative voiding symptoms and hematuria. Nineteen (67.8 %) of the stents were complicated. The complicated stents were managed by a combination of endourological techniques and ESWL. Six (21.5 %) patients presented with renal failure. Three patients died of complications. Forgotten DJ stent can be a lethal yet entirely preventable complication. Preoperative imaging with a noncontrast CT is essential especially in long indwelling time, especially to evaluate stone burden at the upper end of the stent. A stepwise approach should be used for management. Where long operative times are expected, the procedure must be staged.Combined endourological procedures are almost always successful in managing these challenging cases.

BACKGROUND: Ureteral catheters are commonly used by different medical specialties more often in medical centers that have a considerable flow of patients making monitoring difficult. Furthermore, forgotten catheters can cause serious complications increasing hospital costs and the possibility of litigation. This study evaluated a Google Drive-hosted computer control system with color-coded warnings in an attempt to reduce the protracted use of ureteral catheters. METHODS: One month after implanting the program, 119 patients were enrolled in the double J ureteral catheter control system. RESULTS: After the implantation of the system, there was a 16.5% reduction in the length of use of ureteral catheters compared to one year earlier. Importantly, the percentage of patients using double J for six months or more decreased, thus the system has contributed significantly to reduce cases of forgotten catheters. The primary limitation was the relatively small number of patients who require prolonged use of ureteral catheters benefit from the use of this computerized warning system. CONCLUSIONS: This study showed that there was a reduction in the relative risk of forgotten catheters, a shorter mean time of catheter use and a decrease in the number of patients with excessively long catheter use.

Introduction: Double J stent (DJS) is a basic and valuable tool in urological practice. A forgotten DJS is not uncommon and is associated with significant morbidity and mortality. The presenting complaints may range from UTI to renal failure. Therefore, this study has been carried out to assess the clinical spectrum of this entity in our setting. Objective: To describe the clinical presentation, complications and management of forgotten DJS. Methods: This retrospective descriptive study was done at the Department of Urology, Services Institute of Medical Sciences, Lahore (SIMS) from August 2015 to December 2016. Sixteen patients with forgotten DJS were included. Each patient was studied for age, gender, presenting complains, indication of DJS, duration of stent insertion and management performed. The data was recorded on a proforma and analyzed using SPSS version 20. Results: 16 patients 68.8% (n=11) male and 31.2% (n=5) female patients were included in study. The average age was 46.38 ± 13.58 (23-65 years). The mean indwelling time of DJS was 16.31 ± 32 months. 50% of patient's had obstructive uropathy due to stones. Most patients had UTI (37.5%) at presentation while mild encrustation was the major complication encountered (43.8%). Majority of patients were not compliant to the advice for DJS removal. Conclusion: Forgotten DJS leads to multiple complications, if not intervened timely. They can be successfully removed employing least invasive endourologic techniques. Proper counseling of patient/relatives is needed to get it removed in the due time period.
