A Staff Management System for Maintaining Improvements in Continence With Elderly Nursing Home Residents

Abstract: We developed a staff management system for maintaining treatment gains achieved on a specialized continence unit located in a geriatric nursing home. Geriatric assistants learned to use a prompted voiding procedure to maintain improved dryness for 4 elderly residents. The staff management system induded self-monitoring and recording of prompted voiding activities and supervisory monitoring and feedback based on group performance of these activities. Results show that the system was effective in maintaining pro… Show more

“…Although this possibility cannot be dismissed, previous staff training studies suggest that administrative requests alone do not produce lasting changes in staff behavior (Burgio et al, 1990;Quilitch, 1975 The results suggest that generalization occurred in teachers' and children's behaviors after supervisor training. A collaborative team approach is consistent with a "self-mediated stimuli" strategy to encourage generalization (Stokes & Osnes, 1986, p. 433).…”

“…One component was supervisor feedback to teachers about their development and implementation of programs to promote social interaction. Administrative feedback has been shown to be an effective procedure for staff training in institutions (Page et al, 1982), schools (Parsons et al, 1987), and a nursing home (Burgio, Engel, Hawkins, McCormack, & Jones, 1990).…”

“…The daily activities of many practicing behavior analysts frequently involve the oversight of staff responsible for behavior plan implementation. Although the behavioral literature contains successful demonstrations of staff management procedures (e.g., Burgio et al, 1990), many of these involve default procedures such as feedback that are not necessarily matched to the determinants of the performance problem. The PDC-HS may be useful to practicing behavior analysts by (a) helping them understand performance problems that do not respond to simple and quick solutions, and (b) by helping them develop a more sensitive, targeted intervention for those performance problems.…”

“…As just one example, incontinence at day-care centers can promote the spread of infectious diseases (Berk & Friman, 1990). Yet, abundant experimental evidence indicates that with skills training urinary continence can be established and maintained effectively in diverse populations, including infants (Smeets, Lancioni, Paul, & Oliva, 1985), emotionally disturbed adolescents (e.g., Friman & Vollmer, 1995), severely handicapped individuals (Azrin & Foxx, 1971), and very old adults (Burgio et al, 1990). A popular cultural and commercial trend toward increased diaper usage, however, appears to promote convenience over continence by forestalling (e.g., in the young) or even forgoing (e.g., in the handicapped or very old) skills training (e.g., Goode, 1999;Lancaster, 1990).…”
