A Subjective and Urodynamic Assessment of Unilateral Bladder Neck Incision for Bladder Neck Obstruction

Abstract: Thirty-eight patients with bladder neck obstruction were treated by unilateral bladder neck incision. Good subjective and urodynamic results were obtained in 33 patients. Retrograde ejaculation occurred in 16% of the patients on recommencing sexual activity. The value of combined videocystography and pressure flow studies is discussed. The technique is not recommended for patients presenting with acute retention.

“…The remaining 18.7% of the pa tients reported retrograde ejaculation and 12.5% erectile impotence. Although these results seem to be slightly higher than those observed by several authors after unilat eral incisions [8,15], they confirm the results of other studies [10,[16][17][18], We think that the increased rate of retrograde ejaculation in our cases is that the incision peak flow rates in the long term, and patient satisfaction. Our suggestion is in accordance with Soonawalla and Pardanani's [10] findings.…”

“…These results were similar to those reported in the literature [10][11][12], TUIP which was popularized by Orandi [13] is consid ered a moderately invasive treatment method compared with TURP. It can be considered more advantageous than TURP because it is less invasive, easy to learn, results in fewer complications, and is cost effective [8,10,14], In TUIP, which is an easy-to-learn procedure, the incisions can be made unilaterally or bilaterally as described by sev eral authors [8,11,13,[15][16][17]. We could find no report on the effect of the incisions starting from the ureteral ori fices to the verumontanum on the occurrence of vesico ureteral reflux (VUR).…”

“…We found an incidence of retrograde ejaculation of 8.5% which was significantly below the percentage reported by some authors [3, 13, 14] and slightly above the percentage noted by others (table 4). …”

“…Uroflowmetry measurements for such a long observation period have not been previously published. In 1982, Moisey et al [13] reported that 38 of their patients (75%) had normal flow parameters after unilateral BNI for a maximum follow-up of 24 months. Flow rate measurements had not been performed in the study of Waymont et al [6] which included 107 patients.…”

“…This condition can be treated with BNI as either a unilateral procedure (UI), that aims to reduce the rate of retrograde ejaculation or as a bilateral proce dure (BI), that aims to enhance outflow. Earlier reports describe excellent short-term results [1,2,7], However, long-term assessment is lacking in the liter ature. The present study describes the subjective results, morbidity and urodynamic findings and describes the long-term results of uni-and bilateral incision in terms of reoperation rates.…”
