A Systematic Review of the Methods Used to Assess the Association between Appropriate Antibiotic Therapy and Mortality in Bacteremic Patients

Abstract: Studies of the association between inappropriate antibiotic therapy and mortality among bacteremic patients have generated conflicting findings. We systematically reviewed these studies to identify methodological heterogeneity that may explain the lack of agreement. We identified 51 articles that met the inclusion criteria, and we extracted the following data: study design, definition and measurement of variables, and statistical methods. Only 8 studies (16%) defined inappropriate antibiotic therapy as that wh… Show more

“…Adjustment for acute severity of illness has been advocated in prior reviews (6). However, the precise estimation of the impact of appropriate empirical antimicrobial therapy on outcome remains difficult due to the influence of variables other than acute severity of illness and antimicrobial therapy (12).…”

“…Adjustment for the acute severity of illness has been emphasized as an important tool for the examination of the impact of inappropriate antimicrobial therapy in patients with BSI (6). A risk score recently has been derived and validated to predict the outcome of patients at the time of diagnosis of Gram-negative BSI.…”

“…9,18 Therapy was defined as inactive if the blood isolate was resistant to the agent(s) used or in the absence of any antibacterial medications. Empiric therapy was defined as antibiotics administered in the time period before the identification of the blood culture isolate and susceptibility results were available.…”

“…10 In a systematic review of studies evaluating the association between inappropriate antibiotic therapy and mortality among bacteremic patients, measurement of severity of illness without specified the time at which it was measured was considered a source of methodological heterogeneity that may explain conflicting findings. 20 Our study adhered to key recommendations of this review to reduce the effect of potential confounders by assessing severity of illness through the SAPS II score. 21 The present study was designed as a retrospective casecontrol study in which controls were matched to cases according to three variables, including species of infecting organisms, patient's age and SAPS II score in the 24-48h before blood sampling for culture.…”
