A Systematic Review of the Soteria Paradigm for the Treatment of People Diagnosed With Schizophrenia

Abstract: The studies included in this review suggest that the Soteria paradigm yields equal, and in certain specific areas, better results in the treatment of people diagnosed with first- or second-episode schizophrenia spectrum disorders (achieving this with considerably lower use of medication) when compared with conventional, medication-based approaches. Further research is urgently required to evaluate this approach more rigorously because it may offer an alternative treatment for people diagnosed with schizophreni… Show more

“…a safe and well-defined social milieu, structured daily routines, improved social skills, unambiguous communication and trustful, long-lasting therapeutic relationships. For example, the Soteria Project for the treatment of schizophrenia spectrum patients by nonintrusive psychosocial care has yielded equal -and in specific areas better -results than conventional, medication-based treatment approaches [28][29][30] . Although the experiences of estrangement, anxiety and exposure can decrease in terms of intensity, we claim that these subtle experiences at least latently persist as vulnerability traits in this group of patients.…”

“…All of the above would perhaps be of limited interest to modern psychiatry were it not for the systematic and sustained empirical evaluation of the paradigm's efficacy that has been under taken over the almost 40 years since its inception (Calton 2008).…”

“…The aim is to develop a shared experience of the meaningfulness of the resident's individual social context, both current and historical communities with people who received treatment as usual (TAU), meaning hospital admission and treatment with antipsychotic medication. These have generated a considerable secondary literature, with 76 citations currently identifiable (Calton 2008). The US trials found that at 6-week followup there were significant and similar improvements in global psychopathology in both the Soteria group and the TAU group, even though only 24% of the Soteria group had received any antipsychotic medication (and only 16% of these had received drug treatment for longer than a week).…”

“…Cette simple phrase combine un élément parfaitement exact et un autre complètement faux! Sa manière de décrire comment baisser le traitement est plutôt juste quoique sommaire au vu des connaissances récentes [4]. Cela n'empêche pas Podvoll de décrire quelque chose de profondément vrai, par ailleurs évoqué dans d'autres contextes [5]: la présence d'un environnement non stressant permet aux patients de diminuer le dosage de leurs médicaments.…”
