Background Difficulties in communication in intensive care patients receiving mechanical ventilation are a source of stressful experiences and psychoemotional distress. Objectives To examine the association between communication characteristics and psychoemotional distress among patients treated with mechanical ventilation in a medical intensive care unit and to identify factors that may be predictive of psychological outcomes. Methods A total of 65 critically ill patients, extubated within the preceding 72 hours, were included in this cross-sectional study. Data were collected by using a structured interview. Separate regression analysis of data on 3 psychoemotional outcomes (psychological distress, fear, and anger) were used for baseline variables, communication characteristics, and stressful experiences. Results Difficulty in communication was a positive predictor of patients' psychological distress, and length of anesthesia was a negative predictor. Fear and anger were also positively related to difficulty in communication. In addition, the number of communication methods was negatively associated with feelings of fear and anger. Finally, the stressful experiences associated with the endotracheal tube were positively related to feelings of anger. Conclusions Patients treated with mechanical ventilation experience a moderate to extreme level of psychoemotional distress because they cannot speak and communicate their needs. Nurses should be aware of the patients' need to communicate. Decreasing stressful experiences associated with the endotracheal tube and implementing more appropriate communication methods may reduce patients' distress.

Background: The education presented to the clients by the health care personnel can guarantee the well-being of the individuals and stimulate sound health care behaviors in them. Recognizing the barriers to health education enhances the understanding and knowledge of preventive services. Objectives: The present study aimed at investigating the perspective of health care providers about barriers of instructional programs in Iran. Methods: This qualitative study was conducted in the urban health care centers of Sanandaj, West of Iran, in 2016. Using purposive sampling, 15 experienced employees in charge of health care education were included in the study. Data were collected through individual interview and group discussions, observation and field notes, were and analyzed by conventional content analysis. To confirm the accuracy and strength of the data, credibility, confirmability, and transferability components were taken into account. Results:The results of data analysis yielded 5 major categories: (1) health center related problems; (2) lack of preparation for education; (3) clients' problems; (4) personal problems of the personnel; and (5) organizational factors. The participants reported the significance of intra-and intersectoral cooperation and coordination in implementation of health care education programs and the need for further attention from authorities to health care education. Conclusions:The findings revealed that presentation of health care education is a complicated issue, which is influenced by various factors. Also, the success of educational programs in health care centers is not merely an intrasectoral enterprise rather it requires the cooperation of the public media and related organizations, support of the society and attention of families and authorities to health education.

je za 59 % ispitanika komunikacija s medicinskom sestrom važna odnosno vrlo važna. Njih 85 % vjeruje da je komunikacija između pacijenata i medicinskih sestara često ili uvijek prikladna zbog poštovanja, ljubaznosti i povjerenja. Više od 10 % ispitanika već je bilo u sukobima s medicinskim sestrama, koji su riješeni mirnim dijalogom, isprikom, pa čak i uzvišenjem glasa. Međutim, svjesni su da je najbolja aktivnost za smanjenje sukoba strpljenje svih uključenih.

Abstract: Communication with all patients is very important to the provision of quality nursing care. Communication cannot always be achieved using the speech modality. Nurses need to have tools and skills that will allow them to communicate with all of their patients whether or not they can speak.
