A tailored approach to building specialized surgical oncology capacity: Early experiences and outcomes in Malawi

Abstract: ObjectivesCervical cancer can often be cured by surgery alone, if diagnosed and treated early. However, of the cancer patients who live in the world's poorest countries less that 5% have access to safe, effective and timely cancer surgery. We designed a novel, competency-based curriculum to rapidly build surgical capacity for the treatment of cervical cancer. Here we report experiences and early outcomes of its implementation in Malawi.MethodsCurriculum implementation consisted of preoperative evaluation of pa… Show more

“…As of early 2019, there are six trained gynaecology surgeons providing radical hysterectomy surgery across Malawi, totally inadequate to meet current need. A tailored, but resource-intensive approach for rapid building of surgical capacity for radical hysterectomy through a competency-based curriculum and use of master trainers and clinical mentoring has proved effective in Malawi 62 , a model also adopted by others in sub-Saharan Africa. 63…”

“…The primary objective of our initiative was to build surgical oncologic capacity through the implementation and further validation of a training approach designed for rapid surgical skills transfer [10]. Using a non-traditional approach to surgical training that takes advantage of high volumes of disease, we accelerated the usual time required for a trainee to master the skills necessary to perform potential curative cervical cancer surgery.…”
