A variable speed translating couch technique for total body irradiation

Abstract: We have developed a variable speed translating patient couch system for the delivery of total body irradiation (TBI). For a whole body Rando-type phantom, dose variation at mid-plane relative to the prescription point (navel) can be as high as 15% (neck or legs) with a constant velocity. By taking into account variations in body thickness, the intensity modulation radiation therapy, resulting from variable velocities, effectively delivers a uniform dose distribution at mid plane. The couch control user interfa… Show more

“…Large radiation fields are required to cover the entire patient during TBI dose delivery. In some cases, dedicated and specialized TBI facilities are used to cover the entire patient body 2 , 3 , 4 , 5 , 6 . To limit doses to OARs, labor‐intensive physical shielding blocks are usually used (7) …”

“…A translational couch technique maintains a static beam and gantry, while the patient travels under the linac on a specialized translational couch 11. A homogeneous dose can be delivered by varying the speed of the couch movement 12, 13. Custom‐made lung shields and beam spoilers may still be required for these techniques.…”

“…Due to the difficulties accommodating TBI treatments using conventional radiotherapy treatment units, and the associated suboptimal treatment characteristics, some facilities performing large numbers of TBI treatments have devised dedicated TBI units by modifying existing clinical equipment to overcome these shortcomings. Some examples of these units are described in the literature using either linear accelerators2, 3, 4, 5 or Co‐60 units 6, 7, 8…”
