Ab externo approach to treat bilateral haptic extrusion secondary to scleral erosion of an intrascleral sutureless-fixated intraocular lens

Abstract: Sutureless intrascleral fixation of Carlevale intraocular lenses (IOLs) is a new technique that improves previous options for IOL fixation in the absence of capsular support. This lens may be reimplanted in patients with extrusion of the IOL harpoons using a minimally invasive procedure without exchanging the lens.

“…3,5,7 In the postoperative period, both techniques may cause conjunctival erosion, haptic extrusion, and infection. [3][4][5]7,8 With the Yamane technique, IOL haptics are exteriorized from the sclera and then burned to create a "plug." This maneuvre prevents the haptic to slide back into the vitreous chamber through the sclerotomies.…”

“…Similarly to the Yamane technique, the characteristic "t-shaped" harpoons of the Carlevale lens may extrude and damage the conjunctiva. 8 With the PIVC-assisted technique hereby described, it is easy to create sclerotomies of the same length, because the wound construction is straight and not bevelled. The exteriorization of the IOL haptics is relatively simple because they are directly grabbed with the forceps, reducing intraocular manipulation.…”
