ABC of Blood Pressure Measurement: The observer.

Abstract: Apart from the stethoscope, the sphygmomanometer is probably used S tmore than any other single instrument in both hospital and general practice. Doctors, nurses, and paramedical staff record blood pressure; and decisions on treatment, investigation, prognosis, fitness for insurance and employment, and epidemiological conclusions are based on these measurements. More recently patients themselves are being trained to record their own blood pressure, and there are now machines which record

“…33 All the physicians who agreed to participate attended preliminary meetings, under the supervision of central and local coordinators, during which they checked their sphygmomanometers and were trained in the standard methods of blood pressure (BP) [34][35][36][37][38][39][40] and anamnestic recording. Each GP agreed to enroll a random sample of 10 patients older than 64 years who had presented to his or her clinic for any reason: the first eligible patient was chosen each day until recruitment was completed.…”

“…This may be especially important in studies on the elderly because of the wide BP variability in this age group. [34][35][36][37][38][39][40] No other study had the opportunity to verify the prognostic significance of BP based on repeated measurements during such a long period. As expected, BP measurements taken during a long period-1 year-are more predictive than a few measurements taken 1 week apart (see relative risks in Figure 4 in comparison with Figure 2).…”

“…The diagnosis must be established based on readings from the arm with the higher pressure, and the response to therapy monitored long-term using recordings from that arm. 2,3 Up to threequarters of individuals exhibit a difference in BP between right and left arms exceeding 10 mm Hg while seated or supine. Pressure tends to be higher on the right.…”

“…1 Accurate and reproducible measurement of blood pressure (BP) is essential and, to this end, extensive guidelines have been published on the appropriate use of the sphygmomanometer. 2,3 At the initial patient encounter, measurements must be made on both arms. The diagnosis must be established based on readings from the arm with the higher pressure, and the response to therapy monitored long-term using recordings from that arm.…”
