Abstract:To report a rare case of a patient with acute obstructive abdômen with a spontaneous type IV hiatal hernia. Case report: 78 year old woman bedridden and with multiple comorbidities, with a history and exams suggestive of acute obstructive abdomen, whose laparotomy demonstrated a large volume hiatal hernia containing a portion of the stomach and omentum.
