Endovascular repair of abdominal aortic aneuriysms. Experience in 28 patientsBackground: Endovascular repair of aortic aneurysms is less invasive than the traditional surgical approach. Aim: To evaluate the early and mid-term results of aorto-iliac aneurysm endovascular repair. Material and Methods: Analysis of 28 patients aged 59 to 86 years (22 males), subjected to an endovascular repair of aorto-iliac aneurysms between 2007 and 2012. Results: The follow up period of patients ranged from 1 to 65 months. A tri-modular bifurcated prosthesis was installed in 25 patients. A bi-modular bifurcated prosthesis was installed in one patient, an aorto uni iliac prosthesis was installed in one patient; in other patient, a straight endoprosthesis was installed after an abdominal visceral disconnection. In cases of associated iliac aneurysms, unilateral iliac embolization was carried out in seven patients and bilateral embolization in one patient. In one case, the sealing of the hypogastric artery was achieved leaning the prosthetic branch on the hypogastric artery ostium. One patient died 31 days after the operation due to multiple organ failure. One patient died 24 months after surgery due to a gallbladder cancer and other patient died 36 months after surgery due to a rectal cancer. The long-term follow-up showed the presence of type 2 endo-leaks in two patients. The aneurysm decreased in size in 83% of patients after 2 years of follow-up. In five patients, a non-progressive laminar parietal thrombosis inside the endoprosthesis, was observed in the immediate postoperative period. It was initially treated with oral anticoagulants. Conclusions: Endovascular repair of aorto-iliac aneurysms is safe and effective in patients with appropriate anatomical conditions, if a skilled surgical team is available.

Napjainkban a világszerte elterjedt mellkasiaortastentgraft-beültetés hazánkban is egyre nagyobb teret nyer. Eddig a hazai tapasztalatokról átfogó beszámoló nem jelent meg. A módszer elterjedése folyamatban van, az eddigi eredmé-nyek elemzése a további kezelési stratégia optimális meghatározása céljából indokolt. A magyar mellkasi stentgraftműtétek perioperatív eredményeit elemeztük 5 év tapasztalatai alapján. Retrospektív, multicentrikus tanulmányunk keretében adatokat gyűjtöttünk Magyarország minden mellkasi stentgraft-implantációt végző intézményéből. Az adatok jelentése önkéntes volt, 5 év mellkasi aortát érintő stentgraftbeültetési adatait összesítettük. Magyarországon 2012 és 2016 között 131 mellkasiaortastentgraft-beültetést végeztek. A férfiak aránya 67,18%, a betegek átlagéletko-ra 62,80 év volt. Az elvégzett beavatkozás az esetek 25,19%-ában volt sürgető. A betegek kórelőzményében 13,74%-ban szerepelt cukorbetegség. A beavatkozást az esetek 64,89%-ában aneurysma, 17,56%-ában dissectio, 6,87%-ában traumás aortasérülés és 10,69%-ában egyéb betegség indikálta. Az aortadissectiós esetek 73,91%-ban akutak voltak. Aneurysma miatt 16,47%-ban történt akut beavatkozás ruptura miatt. A stentgraftbeültetések 26,72%-ában végeztek kiegészítő revascularisatiós műtétet a supraaorticus ágakon (debranching). A posztoperatív időszakban az esetek 4,58%-ában alakult ki stroke, 1,53%-ában volt szükség átmeneti vesepótló kezelésre, és 2,29%-ában alakult ki bél-ischaemia. Harminc napon belüli reoperációra 5,34%-ban volt szükség. A műtétek 30 napos mortalitása 9,92% volt, az 5 éves utánkövetés során bekövetkezett hosszú távú halálozás 16,03%-ot ért el. A mellkasi aorta stentgrafttal tör-ténő endovascularis helyreállító műtétje hatékony eljárás, előnyei egyértelműek a hazai adatok alapján is a nyitott mellkasi műtétekkel szemben. További hazai elterjedése a centralizált érsebészeti ellátás kialakításának és a megfelelő finanszírozásnak a függvénye. Az optimális kezelés biztosításához elengedhetetlen a társszakmák jelenléte és a megfelelő logisztika biztosítása. Orv Hetil. 2018; 159(2): 53-57. Abstract: The Zenith device is safe, versatile, and effective in the short to medium term. Most patients need wide stent grafts (>or=28 mm proximally and >or=16 mm distally) to achieve 10% to 20% oversizing to prevent type I endoleak.
