Abdominal cocoon: idiopathic sclerosing encapsulating peritonitis

Abstract: SUMMARYAbdominal cocoon, or idiopathic sclerosing encapsulating peritonitis, is a rare condition characterised by the presence of a dense fibrocollagenous membrane partially or totally encapsulating the small bowel leading to recurrent intestinal obstructions. We present the case of a patient who has presented for the fourth time with a small bowel obstruction. Previous laparoscopy revealed a plaque-like reactive process encapsulating much of the small bowel and the liver. After initial adhesiolysis, the patie… Show more

“…Abdominal cocoon syndrome also known as primary sclerosing encapsulating peritonitis is a rare cause of small bowel obstruction [ 2 ]. The exact pathophysiology of the disease remains unknown, although many hypotheses have been made [ 2 , 3 ]. Computed tomography with intravenous contrast is considered the most useful radiological examination for the diagnosis of this abnormality as far as for the decision-making [ 4 ].…”

“…Symptomatic patients with complete small bowel obstruction will require surgical treatment. Excision of the thick fibrous membrane sac with adhesiolysis to release the intestine without intestinal resection is the treatment of choice with good long‐term outcome 4. An alternative treatment with steroids, tamoxifen, colchicine, azathioprine and mycophenolate mofetil has also been described 4…”
