Due to the increased prevalence of obesity in the last decades, bariatric surgery has been on the rise in recent years. Bariatric surgery is a compelling option for weight loss in obese patients with severe obesity-related comorbidities or for whom lifestyle modifications have proven ineffective. Redundant skin following significant weight loss is a common occurrence affecting up to 96% of patients who undergo bariatric surgery, negatively impacting physical and psychosocial health and detracting from activities of daily living. Statistics of the American Society of Plastic Surgeons show that 46,577 body contouring procedures were performed after massive weight loss in the USA in a 2020 report. Abdominoplasty, a well-established cosmetic surgery procedure for improving body contour, is performed by removing excess skin and fat from the abdominal wall and thereby restoring musculofascial integrity and skin elasticity, resulting in a more ideal body shape and increasing quality of life. Although abdominoplasty is a safe procedure, it has been associated with a higher complication rate compared with other body-contouring procedures. Technologic advances over the past decade have been developed as non-invasive alternatives or adjunctive tools to surgery to enhance cosmetic results and minimize complications. New energy-based technologies may supplant invasive surgery for mild to moderate skin laxity and/or diminish the extent of surgery and resulting scars. Plastic surgeons play a significant role in improving the quality of life of patients who suffer from obesity and underwent bariatric surgery. We are deeply convinced, however, that the advancement of knowledge and research in this field will determine the introduction of new technologies and custom-made techniques. This advancement will reduce the complication rate with a rapid reintegration of the patient into the world of work and resumption of daily activities.

Summary: First described by Gaudet and Morestin, abdominoplasty with umbilical preservation dates as far back as 1905. Navel position was described on the transverse axis by Rohrich, and on the median longitudinal axis, by Vernon, Baroudi and Pitanguy. The aim of this article is to validate the 15/10 rule of umbilical positioning in abdominoplasty with the “flap flipping” technique, as an intraoperative aid to simplify umbilical repositioning. Between October 2019 and March 2020, 18 consecutive patients underwent full abdominoplasty, using the 15/10 rule for umbilical positioning. Patients’ average age was 47.9, with body mass index 27.5 kg/m 2 and height 1.63 m. An “expected zone” of umbilical position was delineated by 2 horizontal lines, 15 cm from the bra line and 10 cm from the pubic level. The umbilicus was accurately positioned by using the flap flipping technique before closure of the inferior abdominal incision line. It was then examined to determine whether the umbilicus was situated in the expected zone. In 14 of 18 patients, intraoperative and postoperative measurements of umbilical position were in the expected zone. An average 15.19 cm was measured between the bra line and the new umbilical positioning, and 12.02 cm from the new umbilicus to the pubic inferior suture line. We find that the 15/10 rule of umbilical positioning with the flap flipping technique is an important, intuitive, and easy-to-use guide for precise umbilical positioning in abdominoplasty.

It has become more common among male patients, so clear differences between procedures for men and women have to be stated. The authors present their experience with high-definition lipoabdominoplasty with transverse plication in men. Methods: Records of male patients undergoing transverse plication full abdominoplasty in addition to high-definition liposculpture were analyzed. A total of 24 consecutive cases were found between January of 2017 and June of 2019. Patient ages ranged from 24 to 60 years. Patients aged 18 years or younger were excluded. Body mass index ranged from 25 to 33 kg/m 2 . Photographic records were taken before and during follow-up at 2 days and 1, 3, 6, and 12 months after surgery. Results: Male TULUA (transverse plication, no undermining, full liposuction, neoumbilicoplasty, and low transverse abdominal scar) with high-definition lipoabdominoplasty was successfully achieved in 24 cases. No major complications were reported. Six minor complications were reported (25 percent). Rectus abdominis diastases are less common in men compared to women, as pregnancy is the most determining factor in its development. Fat distribution is also a key difference when performing lipoabdominoplasty for the male or the female patient. The authors recommend a transverse plication of the abdominal wall, instead of a vertical one, as flap viability is preserved and enhanced muscular definition can be accomplished. Conclusions: Combining transverse plication with high-definition lipoabdominoplasty (transverse plication, no undermining, full liposuction, neoumbilicoplasty, and low transverse abdominal scar plus high-definition lipoabdominoplasty) is a safe and reproducible technique for the male patient.
