Ablation and Pacing: Improving Brain Perfusion and Cognitive Function in Patients with Atrial Fibrillation and Uncontrolled Ventricular Rates

Abstract: Patients with atrial fibrillation and rapid ventricular rates refractory to medical treatment have marked signs of brain hypoperfusion and impaired CF. Ablation and pacing improve left ventricular systolic function, thereby increasing BP and improving CF.

“…39 Indeed, restoration of sinus rhythm in patients with AF through cardioversion or ablation leads to improved cerebral blood flow. 40, 41 Reduced diastolic function and low cardiac index, both potential consequences of AF, have also been associated with incident dementia in prospective studies. 42, 43 Finally, shared risk factors, such as hypertension, coronary disease, and diabetes, which tend to accumulate with ageing,…”

“…Left ventricular ejection fraction was substantially normal before implantation (62.5%±8.4) and significantly ameliorated after implantation (66.2%±8.2). 24 Another hypothesis foresees AF as a factor associated with a proinflammatory and prothrombotic state, which is reflected by upregulation of various blood indices, including prothrombotic markers, platelet activation, inflammatory markers, blood stasis within the atria, and ultimately by the presence of endothelial dysfunction, both at cardiac and vascular level. 3,25 Circulating biomarkers, particularly those related to endothelial function, have been proposed as useful tools for a better comprehension of AF pathophysiology, as well as for refining the clinical risk stratification scores currently used for therapeutic decision making.…”

“…Ранее опубликованные нами данные [7,9,10] и результаты, полученные в настоящем исследова-нии, позволяют утверждать, что именно нарушение церебральной перфузии является одной из важней-ших причин развития нейрокогнитивного дефицита у больных МС. Отдельные составляющие метаболи-ческого синдрома сами по себе могут оказывать нега-тивное влияние на церебральную перфузию и, как следствие, на когнитивную функцию.…”
