Absorption Kinetics of Berberine and Dihydroberberine and Their Impact on Glycemia: A Randomized, Controlled, Crossover Pilot Trial

Abstract: Berberine is a natural alkaloid used to improve glycemia but displays poor bioavailability and increased rates of gastrointestinal distress at higher doses. Recently, dihydroberberine has been developed to combat these challenges. This study was designed to determine the rate and extent to which berberine appeared in human plasma after oral ingestion of a 500 mg dose of berberine (B500) or 100 mg and 200 mg doses of dihydroberberine (D100 and D200). In a randomized, double-blind, crossover fashion, five males … Show more

“…The observed AUC in the placebo, B500, D100, and D200 was 20.2 ± 16.2, 42.3 ± 17.6, 284.4 ± 115.9, and 929 ± 694 ng/ml respectively. These results indicate that the plasma concentration of dehydroberberine is remarkably higher than those observed in berberine administration and placebo conditions (Moon et al, 2022).…”

“…It was found that the demethylation of berberine enhances its intestinal absorption in mice in vivo, which increases the therapeutic potential of berberine [ 29 ]. Furthermore, dihydroberberine is considered an active metabolite as it showed up to five times more bioavailable/absorbable and two times longer-lasting than berberine (8 h vs. 4 h) [ 30 ]. Overall, these findings further support microbial models of drug metabolism in mammals.…”

“…Although berberine seems convincing as a promising antidiabetic, the temporary adverse gastrointestinal events observed in one third of patients treated with the alkaloid in a trial of 59 patients [ 220 ], has restrained the medical community from using it for clinical purposes. This worry is further supported by the inconsistent bioavailability of berberine after oral ingestion, as shown by a randomized, double-blind, placebo-controlled investigation [ 221 ]. In addition, as mentioned above (see Section 2.3 ), there are conflicting results of the effect of berberine on insulin secretion [ 130 , 132 ].…”
