Laryngeal cancers constitute 2-5% of all cancers, and 45% of all head and neck cancers. Acantholytic squamous cell carcinoma (ASCC), also known as adenoid squamous cell carcinoma, is a rarely observed histological subtype of squamous cell carcinoma (SCC). While they are rarely seen in the digestive and respiratory tracts, their course tends to be more aggressive. Furthermore, while head and neck squamous cell carcinoma tend to have a low rate of distant metastasis, the lungs are still the most common target organs involved by metastasis. Metastasis from the laryngeal region to soft tissues, skin and adrenal areas is rarely encountered.

Larinksin malign epitelyal tümörleri arasında skuamöz hücreli karsinom en sık görülendir. Dünya Sağlık Örgütü, skuamöz hücreli karsinomun farklı alt tiplerini tanımlamıştır. Bu varyantlardan bazıları konvansiyonel skuamöz hücreli karsinoma göre daha iyi, bazıları ise daha kötü prognozludur. Çalışmamızda skuamöz hücreli karsinomun az görülen varyantları sunulmaktadır. Gereç ve Yöntem:Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı'nda son 7 yıllık dönemde tüm larinks biyopsileri taranmıştır. Skuamöz hücreli karsinom varyantları şeklinde tanı alan hastalar belirlenmiş, olguların histopatolojik ve klinik özellikleri derlenmiştir.Bulgular: Yedi yıllık dönemde larinks biyopsisi yapılan 1148 olgu bulunmaktadır. Bu olgulardan 460'ı skuamöz hücreli karsinom tanısı almıştır. Bunlardan 31'i nadir görülen skuamöz hücreli karsinom varyantıdır. Bunlar arasında 3 verrüköz karsinom, 6 bazaloid, 11 papiller, 4 iğsi hücreli, 6 akantolitik varyant skuamöz hücreli karsinom olgusu mevcuttur. Bir olgu ise adenoskuamöz karsinom tanılıdır. Verrüköz karsinom olgularında tümör vokal kord yerleşimlidir. Bazaloid varyant ise olguların tümünde supraglottik bölgede saptanmıştır. Diğer varyantlarda glottik ve supraglottik bölge değişen oranlarda tutulmaktadır. Olguların sadece birinde infraglottik tümör mevcuttur. Bu olgu akantolitik varyant skuamöz hücreli karsinom olarak değerlendirilmiştir.Sonuç: Larinksin skuamöz hücreli karsinomunun az görülen varyantlarının biyolojik davranışları farklılıklar göstermektedir. Bu tümörler multidisipliner yaklaşım ile efektif şekilde tedavi edilebilir. Alt tiplerin tanınması ve tariflenmesi uygun tedavi yaklaşımının belirlenebilmesi için gereklidir.

In the last decade, the incidence and mortality associated with laryngeal malignancies has experienced an unfavorable evolution in Romania, in terms of this pathology the country that has become the leader, among the European Union (EU)-27 countries. The aim of the present retrospective study was to analyze the epidemiological data regarding laryngeal cancer and histopathological (HP) particularities in patients diagnosed in Western Romania. Within three years and 10 months (during October 2016-July 2020) in Ear, Nose and Throat (ENT) Clinic of Timişoara Municipal Emergency Clinical Hospital, 194 patients with laryngeal neoplasms (LN) were diagnosed. Most of the patients were male (93.8%), from Timiş County, mainly and the neighboring Counties of Caraş-Severin, Hunedoara, Mehedinţi and Arad. Patient's average age (both sexes) was 62.6 years, with a range from 38 to 84 years and the highest percentages in the 60+ age group (48%). Regarding smoker status, the main cause of the development of the LN, 93.9% of patients were active ones. Regarding localization [International Classification of Diseases 11 th Revision (ICD-11)], about 69% were malignant neoplasm of glottis (C32.0), followed by supraglottis (C32.1), larynx-unspecified (C32.9), and subglottis (C32.2). From the HP point of view, several types of tumors were identified, most of which were non-keratinized and keratinized squamous cell carcinomas (about 90%). In addition, there were identified types of verrucous carcinoma, acantholytic carcinoma, squamous papilloma, a possible pleomorphic sarcoma, and pyogenic granuloma. The data presented in this study highlight the growing incidence of LN, which mainly affect men, at an increasingly young age. It is imperative to involve specialists from nutrition, general medicine, ENT, dentistry to start awareness programs and to develop prevention protocols.
