Acanthosis nigricans Caused by Nicotinic Acid: Case Report and Review of the Literature

Abstract: Drug-induced acanthosis nigricans has been reported in the literature. We present a patient with familial combined hyperlipidemia who developed nicotinic-acid-induced acanthosis nigricans. The literature on the cutaneous side effects of nicotinic acid as well as on the medications that can cause acanthosis nigricans is reviewed. Some hypotheses on the pathogenesis of nicotinic-acid-induced acanthosis are presented.

“…Most widely recognized is the association between nicotinic acid [64][65][66] and AN, which typically develops on the abdomen and flexor surfaces of the skin and resolves within 4 to 10 weeks of stopping the medication. 66 Patients with insulin-requiring diabetes may acquire AN as a result of repeated injection of the same sites.…”

“…The mechanism of niacin's effect on the macula is unknown (127). Acanthosis nigricans (128) or niacin-induced myopathy (129) belongs to other uncommon adverse effects of niacin. Niacin ER monotherapy has specifically amassed considerable clinical trial data that do not support an increased risk for muscle adverse experiences with niacin monotherapy (130).…”
