Acceptability and feasibility of the use of 400 μg of sublingual misoprostol after mifepristone for medical abortion up to 63 days since the last menstrual period: Evidence from Uzbekistan

Abstract: Mifepristone medical abortion with home administration of misoprostol is an acceptable and feasible option for women in Uzbekistan. The method worked well at all three facility types.

“…Our baseline assumptions for sample size included an ongoing pregnancy rate of 1.2% based on previous studies [16,17]; however, the ongoing pregnancy rate in this study was lower among women assigned to both phone and clinic follow-up, and this fact may have reduced our statistical power to detect any differences. In addition, our study did not require all women to return to the clinic to verify their abortion outcome.…”

“…We estimated (with α= 0.05, 1-β=0.8, with a one-tailed test) that at least 14 ongoing pregnancies should occur per group. Based on previous data from these facilities, we estimated that ongoing pregnancy would occur in about 1.2% cases of medical abortion [16,17]; therefore, we would be required to enroll approximately 1167 women in each group. Finally, to account for loss to follow-up, we rounded up to 1200 women in each group, with a total sample size of 2400 women.…”

“…The woman should understand how much bleeding is considered too much. Women can safely and effectively self-administer misoprostol at home as part of a medical abortion regimen (32,34,35). Buccal and sublingual misoprostol can be administered as early as 24 hours after mifepristone administration (32,33).…”

“…Extensive evidence regarding both efficacy and acceptability supports the outpatient use of medical abortion through 63 days since the last menstrual period (LMP). [1][2][3][4][5][6] In this pregnancy range, among the World Health Organization's (WHO) recommended regimens is 200 mg mifepristone followed by 800 μg buccal misoprostol; 7 this protocol is routinely used for legal abortion in the outpatient facilities of the Secretariat of Health of Mexico City (SSDF), 6,8 where approximately 71% of women who obtain services have a medical abortion. 9 In Mexico, abortion is legal in the first trimester of pregnancy, and the first mifepristone product was registered there in 2011.…”
