Accidental, Non-masturbatory, Non-intercourse Related, Self-Inflicted Penile Fracture: Case Report and Review of Literature

Abstract: Penile fracture is a relatively rare condition arising from a rupture of the corpus cavernosum. In the western world, it is most commonly associated with sexual intercourse. We hereby, report a rare case of accidental, non-masturbatory, self-inflicted penile fracture. It was diagnosed promptly based on clinical assessment, confirmed radiologically and managed surgically, resulting in a good long-term clinical outcome. Our case highlights the importance of maintaining a high index of suspicion to diagnose this r… Show more

“…These injuries are predominantly the result of trauma during sexual activity, with vaginal intercourse being cited as the most common cause. However, other mechanisms have been identified, including masturbation, accidental rolling during sleep, and even self-manipulation [ 6 , 7 ]. Risk factors exacerbating the probability of such fractures include the use of pharmaceutical agents that prolong erection, existing urethral or periurethral infections, and previous penile trauma [ 8 ].…”

“…For example, in the majority of cases in the USA, penile fracture occurs during vigorous sexual intercourse; however, “taghaandan,” a common practice in Middle Eastern countries of bending the penis until an audible click is heard, has been reported in up to 60% of cases in one large series. 16 17 Nevertheless, our study benefits from its large sample size over nearly one decade. The use of a control cohort of surgical patients allowed us to compare to a similar yet general population.…”

“…The underlying pathology of penile fracture is rupture of the corpus cavernosum of an erect penis (8, 9). During erection, the tunica albuginea is stretched / thinned, and if put under strain by manipulation, the firmly engorged corpus breaks through, leading to penile fracture.…”
