The complex and evolving landscape of drug addiction poses significant public health challenges in Algiers, Algeria. With growing concerns about drug addiction and its associated consequences, it becomes imperative to comprehensively understand drug consumption patterns among individuals grappling with addiction in this region. Drug addiction is a multifaceted issue influenced by various factors such as drug availability, sociodemographic characteristics, and personal choices. The prevalence of drug addiction continues to rise, warranting a thorough examination of the specific substances that dominate the local addiction landscape. A descriptive retrospective analytical study was conducted, analyzing 92 cases of drug consumption profiles among addicts in the Algerian province of Algiers. Urine samples collected between January 1, 2020, and October 15, 2022, from drug-addicted patients in Algiers, were examined using Gas Chromatography-Mass Spectrometry (GC-MS). The meticulous analysis of this diverse population revealed that THC and pregabalin are the two most frequently consumed substances, often used together in poly-drug combinations. Surprisingly, there is a notable prevalence of opioid consumption, especially within families, raising concerns about a potential opioid crisis in Algiers.

Background In clinical toxicology, laboratories need screening methods allowing unambiguous identification of the compounds in a short turnaround time to either confirm or exclude the hypothesis of drug overdose, or poisoning with a toxicant. Methods We developed a fully automated screening procedure designed to identify and quantify in a single run 245 compounds of interest in clinical toxicology. Sample extraction was carried out by a programmable liquid handler directly coupled to a liquid chromatography–tandem mass spectrometry (LC–MS-MS) system. Data acquisition was performed in the positive and negative ionization modes with up to 15 multiple reaction monitoring (MRM) transitions per compound, each with optimized collision energy to enable both qualitative library searching and quantitation. The method was validated according to the ISO 15189 requirements and was applied to real patient samples (n = 127). Results The 15 MRM transitions per compound provided higher confidence for the identification of all the compounds. The quantitative method was fully validated with satisfactory intra- and inter-assay imprecision and inaccuracy with CV% lower than 20%. For only 9 molecules, imprecision and inaccuracy were relatively high but never exceeded 31.7%. Comparison with dedicated quantitative methods using conventional MRM monitoring performed using 127 patient samples (n = 175 pairs of measured concentrations) showed excellent correlation (R2 = 0.96). A robustness study showed that calibration curves prepared for up to one month yielded uncertainty < 20%. Retention times ranged from 0.89 min for metformin to 9.72 minutes for difenacoum. The automated sample preparation required 8 minutes and was followed by 10 min chromatographic separation. Conclusion This first-line screening procedure yields high confidence in compound detection and should be useful in core labs facing clinical toxicology situations where rapid and reliable results are needed.

N-Ethylhexedrone: A very long and bad trip! A case series

N-ethylhexedrone (NEH) is a new cathinone derivative with, currently, low toxicocokinetic, and toxicocodynamic knowledge. We present 3 documented clinical cases of NEH intoxication with plasma, and urine concentrations. A thorough search for metabolites was performed. The 3 patients were admitted to the emergency department, and 2 out of the 3 were hospitalized for an extended period. While recovering from the drug effects, 12 to 24 hours after nasal intake of New Psychoactive Substance (NPS), the patients described the following disorders: anxiety, feelings of persecution, asthenia, anhedonia, abulia, psychomotor slowing, and loss of consciousness. NEH was identified in all samples by liquid chromatography–high resolution mass spectrometry (LC–HRMS), and quantified by liquid chromatography coupled to tandem mass spectrometry (LC–MS-MS). Quantitative analysis showed decreasing concentrations over time: for case 1, from 97.2 (Day 1, D1) to 0.7 (Day 7, D7) µg/L for plasma, and from 724 (D1) to 0.5 (D7) µg/L for urine. NEH concentration of 7.9 µg/L was found in the plasma collected at admission for case 2. For case 3: concentrations ranging from 49 (D1) to 1.8 (D7) µg/L in plasma, and from 327.3 (Day 5, D5) to 116.8 (D7) µg/L in urine were found. NEH was no longer detected in the urine sample at Day 10. Elimination half-life was estimated at 19, and 28 hours in patients 1 and 3, respectively. Four metabolites were identified in blood and urine: reduced NEH, dealkyl-NEH, reduced dealkyl-NEH, and hydroxy-NEH. The cases presented highlight the long detectable lifetime of NEH. Characterization of the metabolites will allow better identification of the consumption of this drug. Serious adverse events can be observed after NEH consumption, as 2 out of 3 patients required intubation and ventilation. A syndrome of inappropriate antidiuretic hormone secretion (SIADH) was also diagnosed. Two out of the three cases are notable because of the number of samples collected and because NEH was the only drug of abuse detected.
