Accuracy of the Clinical Diagnosis of Corticobasal Degeneration

Abstract: The accuracy of the clinical diagnosis of corticobasal degeneration (CBD) is unknown. To determine its diagnostic accuracy, we presented 105 cases with known neuropathologic diagnoses, including CBD (n = 10), progressive supranuclear palsy (PSP, n = 24), Parkinson's disease (n = 15), diffuse Lewy body disease (n = 14), multiple system atrophy (n = 16), postencephalitic parkinsonism (n = 7), Pick's disease (n = 7), Creutzfeldt-Jakob disease (n = 4), Alzheimer's disease (n = 4), vascular parkinsonism (n = 3), an… Show more

“…This case report confirms the low diagnostic sensitivity in predicting CBD, which is consistent with the literature and shows the broad clinical spectrum that CBD exhibits [1,2]. CBD can be regarded as one of the mentioned distinct degenerative entities and these diseases can also masquerade as CBD.…”

“…Corticobasal syndrome (CBS) is a progressive disease that includes a heterogeneous neuropathological spectrum comprising corticobasal degeneration (CBD), Alzheimer disease (AD), progressive supranuclear palsy (PSP or Richardson's syndrome) and frontotemporal lobar degeneration, Creutzfeldt-Jacob disease, and vascular degenerative diseases [1][2][3]. In the majority of cases, CBD is the underlying disease.…”

“…However, patients presenting with a motor syndrome have been shown to develop aphasia or behavioral symptoms later in the disease course (Kertesz et al, 2000) and cases initially showing relatively isolated speech and language, visuo-spatial and behavioral disorders have been described (Lang, 1992;Bergeron et al, 1996;Mathuranath et al, 2000;Mimura et al, 2001;Kertesz and Munoz, 2003;Tang-Wai et al, 2003). The variability in cortical and basal ganglia involvement in CBD remains a source of diagnostic confusion (Litvan et al, 1997;Boeve et al, 1999).…”

“…In particular, the phenotypes of the frontotemporal dementias (FTD), 17,18 vascular dementia syndromes, 19,20 and dementia with Lewy bodies (DLB) 21 are not easily or well-identified with DSM dementia criteria. 22 For each of these disorders there have been recently operationalized criteria proposed through consensus meetings with resultant publications that are allowing the phenotypes to be tested. [23][24][25][26][27] As some of these disorders require primary or secondary treatment interventions and differential pharmacological management, familiarity will be imperative for primary care physicians and specialists who are involved in the care of dementia.…”
