Accurate Detection of Streptococcus pyogenes at the Point of Care Using the cobas Liat Strep A Nucleic Acid Test

Abstract: The performance of a polymerase chain reaction-based point-of-care assay, the cobas Strep A Nucleic Acid Test for use on the cobas Liat System (cobas Liat Strep A assay), for the detection of group A Streptococcus bacteria was evaluated in primary care settings. Throat swab specimens from 427 patients were tested with the cobas Liat Strep A assay and a rapid antigen detection test (RADT) by existing medical staff at 5 primary care clinics, and results were compared with bacterial culture. The cobas Liat Strep … Show more

“…Culture was used as the gold standard of comparison, with RT-PCR also being performed for adjudication in cases where the results of all 3 tests (Alere i, BD Veritor, culture) did not agree. While other studies comparing molecular POCT methods to a mixed group of rapid antigen tests, culture (12), and a lab-developed test (13) and Alere i to culture (14) have been performed, to our knowledge, this is the first study comparing Alere i and a rapid antigen test head-to-head and analyzing the hypothetical impact on antibiotic utilization based on the test results.…”

“…Similarly, the role of gram-negative bacteria should be investigated. The role of rapid detection systems for Group A Streptococcus has been validated (Gazzano et al, 2016;Hudson, Theron, Roditi, & Bloch, 1991;Wang et al, 2017) and we feel there could be a role for such kits in cleft palate surgery.…”
