Acinetobacter baumannii is a Gram-negative bacterium that has gained a stronghold inside healthcare settings. Due to the ability of A. baumannii to acquire antibiotic resistance easily, its presence in food products could pose a major threat to the public health. The aim of this study therefore, was to investigate the prevalence of A. baumannii in fresh produce and study their genetic diversity. A total of 234 samples of vegetables and fruits were collected. A. baumannii isolates were identified using CHROMagar and two different PCR assays. Also, the isolates were tested for their ability to resist antibiotics and form biofilms. The genetic diversity of the isolates was determined using multi-locus sequence typing (MLST). Of the 234 samples collected, 10 (6.5%) and 7 (8.75%) A. baumannii isolates were recovered from vegetables and fruits, respectively. Antibiotic susceptibility testing revealed that 4 of these isolates were extensively drug-resistant (XDR). All isolates were able to form biofilms and MLST analysis revealed 6 novel strains. This study demonstrated that fresh produce constitutes a reservoir for A. baumannii, including strong biofilm formers and XDR strains. This represents a significant concern to public health because vegetables and fruits may serve as a vehicle for the spread of A. baumannii and antibiotic resistance into the community and healthcare settings.

Acinetobacter baumannii (Acb) adalah salah satu bakteri gram negatif oportunis yang ada di lingkungan. Karbapenem merupakan salah satu agen antibiotik yang digunakan untuk pengobatan infeksi Acb. Laju resistensi Acb terhadap karbapenem dalam beberapa tahun terakhir selalu mengalami peningkatan dengan prevalensi di seluruh dunia mencapai 30%. Penelitian ini bertujuan untuk melihat hubungan kejadian infeksi Carbapenem Resistant Acinetobacter baumannii (CRAB) dengan penggunaan antibiotik karbapenem di rumah sakit St. Carolus. Metode penelitian ini yaitu observasional analitik dengan desain studi case control. Penelitian dilakukan di Rumah Sakit St. Carolus pada bulan Juni-Agustus 2019. Jumlah sampel dalam penelitian ini adalah 149 pasien (110 pasien terinfeksi CRAB, 39 pasien terinfeksi Carbapenem Sensitive Acinetobacter baumannii (CSAB)). Data yang diperoleh dianalisis secara univariat, bivariat dan multivariat menggunakan SPSS versi 22. Analisis multivariat dilakukan dengan analisis regresi logistik etiologik. Hasil analisis multivariat penelitian ini adalah penggunaan antibiotik karbapenem pada pasien dengan riwayat masuk Intensive Care Unit (ICU) memiliki peluang meningkatkan risiko CRAB (odds ratio (OR)=32,266; p=0,020). Penggunaan antibiotik golongan karbapenem paling tidak 24 jam sebelum pengambilan spesimen kultur Acb pada pasien yang menerima perawatan di ICU dalam periode inap merupakan faktor risiko terjadinya infeksi CRAB menjadi lebih tinggi.

Can Drug Repurposing be Effective Against Carbapenem-Resistant Acinetobacter baumannii?

Carbapenem-resistant Acinetobacter baumannii has been classified as a top priority for the development of new therapies due to its resistance to most antibiotics. Drug repurposing may be a fast and inexpensive strategy for treating this pathogen. This review aims to critically evaluate repurposed drugs for the treatment of infections caused by carbapenem-resistant A. baumannii , correlating their antimicrobial activity with data available for toxicity and side effects. Some drugs have been suggested as promising candidates for repurposing; however, in some cases, high toxicity and low plasma concentrations reduce applicability in clinical practice. The most favorable applicability is offered by fusidic acid and colistin, possibly combined with a third agent, promising to be well tolerated and achieving satisfactory plasma concentrations.
