Acral lentiginous melanoma: histopathological prognostic features of 121 cases

Abstract: This is a detailed pathological study of a large cohort with ALM, an uncommon subtype of melanoma. Mitotic activity appears to be of particular importance in predicting the outcome of ALM.

“…Several lines of clinical and epidemiological evidence suggest that acral and common cutaneous melanomas are distinct entities. Thus, unlike common cutaneous melanoma, acral melanoma is not associated with naevi, family history, or known melanoma-susceptibility genes (Phan et al 2007). Acral melanoma is the predominant melanoma subtype in non-Caucasians and has a notably worse prognosis than common cutaneous melanoma (Bradford et al 2009).…”

“…24 Acral lentiginous melanoma is similarly thought to evolve primarily de novo, as there are few reports of ALM with a preexisting lesion. 5,26 In a large cohort of 121 acral lentiginous melanomas, remnants of a nevus were found in just 3% of cases. 5 Other reports have suggested pre-existing nevi in 10-13% of ALM.…”
