Introduction. Acute kidney injury (AKI) following surgical myocardial revascularization is associated with high mortality and morbidity. The aim of this study was to evaluate the risk of acute kidney injury in a population of very old patients following different surgical techniques. Patients and Methods. A retrospective study of 310 consecutive patients aged 78 to 93 years, mean 80.5±2.2, who underwent surgery at one cardiac surgery centre. Based on the surgical technique used the patients were divided into: Group I. CABG (n=134) -surgical myocardial revascularization using extracorporeal circulation and arterial and venous grafts. Group II. OPCABG (n=55) -surgical revascularization without extracorporeal circulation but using arterial and venous grafts. Group III. NOTOUCH (n=121) -no handling with the ascending aorta was performed at all. Results. A statistically insignificant renoprotective trend was found in patients who underwent surgery without extracorporeal circulation regardless of technique. Comparing groups II and III vs. group I, a significantly poorer renal functioning (median difference in creatinine was 10.0 (32.9) vs 17.5 (35.0), P=0.05) was shown for patients in group I. Conclusion. Surgical myocardial revascularization without extracorporeal circulation in very old patients is safe. The results of this study show a renoprotective trend.

Background: The protective effects of off-pump (OPCAB) compared to conventional (CCAB) coronary artery bypass graft surgery on neurological injury and inflammation has been controversial. We evaluated pre-and postoperative levels of the brain injury marker, S100β, and markers of inflammation, Interleukin-6 (IL-6) and high sensitivity C-reactive protein (hs-CRP) in a prospective randomized controlled trial. Methods:A sub-group sample of the randomized controlled trial of 50 consecutive randomized patients (n=27 CCAB, n=23 OPCAB) was utilized for the present biomarker analysis. Each patient had blood drawn before and after surgery. Analysis of variance and Kruskal-Wallis were used to assess significant differences in biomarkers.Results: There was no difference in post-procedure S100β (p=0.1) or change in S100β from baseline (p=0.9). Hs-CRP and IL-6 were higher in the OPCAB arm post-procedure (P CRP =0.001; P IL-6 =0.053) and change from baseline (P CRP =0.003; P IL-6 =0.001). Conclusion:OPCAB did not result in preventing neurological injury over CCAB; however, OPCAB had significantly more inflammation than CCAB following the procedure.

Worldwide, especially in developed countries, prolonged life expectancy results in increasing number of age-related / associated diseases like diabetes mellitus, cardiovascular diseases, malignancies, neurological diseases, organ failures etc. in the community. Beside the numbers, planning and applying the treatment of pulmonary and cardiovascular diseases simultaneously in such patients is stil a major challenge. The knowledge and experience gained from the increasing number of patients all over the world led to the conclusion that it could be possible to treat some patient groups who would be considered as difficult or impossible to cure in the past. In this paper we evaluated the treatment process in lung diseases (especially malignancies) and cardiovascular disease seen simultaneously.Keywords: Lung cancer, heart disease, coronary artery disease, pulmonary surgery, concurrent heart lung disease, concurrent heart lung surgery ÖzetÖzellikle batıda olmak üzere tüm dünyada uzayan ortalama ömür süresi, yaşa bağlı/ilintili hastalıkların da toplumda daha sık görülmesi sonucunu doğurmaktadır. Bunlar diyabet, kalp damar hastalıkları, maligniteler, nörolojik hastalıklar, organ yetmezlikleri gibi hastalıklardır. Bu durumun yarattığı sorunlardan biri de eş zamanlı olarak görülen akciğer ve kalp damar hastalıklarının tedavisinin planlanması ve uygulanmasıdır. Tüm dünyada giderek artan sayıdaki hastalardan elde edilen bilgi ve deneyimler eskiden çok zor hatta imkansız olarak kabul edilen hasta gruplarında bile tedavi seçenekleri olabileceğini ve başarıyla uygulanabileceğini göstermektedir.
