Active treatment of ganglia in children

Abstract: SummaryOne hundred and twenty-eight children, admitted for active treatment of ganglia around the wrist, hand, ankle or foot are reviewed. The methods of treatment are discussed. The most satisfactory results were obtained by excision (7-2% recurrence) and incision with a tenotome (11% recurrence). The overall recurrence rate was 19%. The ganglia around the wrist had the highest rate of recurrence.

“…The fact that the patient had a history of a swelling in the dorsum of the foot that disappeared, only to later recur, was suggestive of a ganglion cyst, because these lesions are known to occasionally disappear spontaneously, frequently recurring some time later (MacKinnon and Azmy, 1977;Colen and Rengachary, 2006). The location of the ganglion between the medial and intermediate cuneiform bones proved to be consistent with the anatomical reasoning presented above.…”

“…The rate of recurrence of 14% determined here is lower than the rate measured in other studies. As mentioned above, it has been measured as high as 43% recurrence rate after aspiration, puncture, or indwelling suture [4]. However, in these cases from the literature review, there was no injection of triamcinolone acetonide.…”

“…Recurrence rate among children is higher than in adults. It has been measured as high as 43% recurrence rate after aspiration, puncture, or indwelling suture [4]. Most are usually managed by observation.…”

“…4 Notably, recurrence rates are reportedly higher for children than adults, ranging from as low as 6% to as high as 35%. 6,8 Suggested surgical maneuvers to decrease ganglion recurrence include excision of cysts down to the stalk, excision of a cuff of ligament, or splinting for immobilization of the wrist. This study focuses on a single surgeon's use of all of these techniques in a solely pediatric population.…”
