Actual 10-Year Survival After Resection of Colorectal Liver Metastases Defines Cure

Abstract: Patients who survive 10 years appear to be cured of their disease, whereas approximately one third of actual 5-year survivors succumb to a cancer-related death. In well-selected patients, there is at least a one in six chance of cure after hepatectomy for CLM. The presence of poor prognostic factors does not preclude the possibility of long-term survival and cure.

“…However, it should be pointed out that there are no randomised studies of metastasectomy in any setting [24][25][26]35]. A further limitation is the short duration of follow-up-11 of 18 studies reported 5-year survival data, but new data are emerging to suggest that, in the case of liver metastases at least, 10-year survival may a more pertinent measure, as cancer-related death only appears to plateau after 10 years [45,46].…”

“…1-3 More than 50% of CRC patients develop metastatic disease to their liver over the course of their life, 4 and liver metastasis is a leading cause of death from CRC. 5-7 Unfortunately, surgical resection of isolated liver metastases of CRC (LM-CRC) is curative in only 20%-30% of patients, 8 , 9 and systemic therapy provides limited survival benefit. 10 Patients with unresectable LM-CRC have a poor prognosis with a median survival of only two years.…”

“…Surgical resection has been recommended in carefully selected patients 4,5 with 5-year survival rates of 30-60% in different studies. [6][7][8][9][10] Most of the experiences describe the role of surgery in liver metastases from colorectal cancer. On the other hand, we have several reports about liver metastasectomy in metastatic BC but these series included small cohort of patients widely different for inclusion criteria and type of systemic therapy received; the reported 5-year survival rate varies from 18% to 61%.…”
