Acute and critically ill peripartum cardiomyopathy and 'bridge to' therapeutic options: a single center experience with intra-aortic balloon pump, extra corporeal membrane oxygenation and continuous-flow left ventricular assist devices

Abstract: IntroductionPeripartum cardiomyopathy (PPCM) patients refractory to medical therapy and intra-aortic balloon pump (IABP) counterpulsation or in whom weaning from these therapies is impossible, are candidates for a left ventricular assist device (LVAD) as a bridge to recovery or transplant. Continuous-flow LVADs are smaller, have a better long-term durability and are associated with better outcomes. Extra corporeal membrane oxygenation (ECMO) can be used as a temporary support in patients with refractory cardio… Show more

“…Inotropes, intra-aortic balloon pumps, LV and biventricular assist devices, and extracorporeal membrane oxygenation should be considered in these cases and have been used successfully. [75][76][77][78][79] Aggressive treatment should generally be sought in light of the frequent recovery of patients with PPCM. When recovery does not occur, cardiac assist devices can serve as a bridge to transplantation.…”

“…48,50 Extracorporeal membrane oxygenation can also serve as a bridge to left ventricular assist devices in patients with refractory cardiogenic shock despite use of an intraaortic balloon pump and full inotropic support. 51 Women in whom maximal medical management is unsuccessful may be candidates for cardiac transplantation. 48 According to one study, 14 cardiac transplantation was necessary in 4% of women with PPCM.…”

“…Peripartum cardiomyopathy is a rare disease affecting women in the last month of their pregnancy or up to five months after delivery; it affects one in every 3000 to 4000 live births in the US each year [1]. It was first described by Demakis in year 1971 [2].…”

“…Extra corporeal membrane oxygenation (ECMO) can be used as a short term temporary support for emergency rescue in patients with refractory cardiogenic shock. Only few reports exist about the place of LVAD and ECMO in severe refractory cardiomyopathy [1].…”
