Acute Cellular Rejection in Heart Transplant Patients: Insights of Global Longitudinal Strain, Myocardial Work, and an Exclusive Group of Chagas Disease

Abstract: BackgroundEchocardiographic markers associated with asymptomatic acute cellular rejection (ACR) in patients with orthotopic heart transplant (HT) are still under investigation. The aim of our study was to determine clinical and myocardial strain imaging (MSI) variables evaluated by echocardiography associated with ACR in the first year of HT. A separate analysis was performed to compare variables during the first 6 months of HT, when ACR has a prevalence in 60% of patients. Another analysis evaluated an exclus… Show more

“…Thus, it comes as no surprise that as the degree of myocyte damage increases, the values for LVGLS and LVGCS decrease. It should be noted that segmental LV longitudinal strain has been shown to be most sensitive when assessing for lower grades of ACR; this can be partly explained by the patchy nature of ACR [17]. However, the scope of this paper did not assess the use of segmental strain values or directly compare prognostication abilities among the various forms of global strain.…”

“…Moreover, ACR positive patients may be clinically asymptomatic or present with non-speci c symptoms. These subtle presentations can often go unnoticed for an extended period, potentially leading to delays in the initiation of essential treatments and, consequently, contributing to unfavorable outcomes [16,17]. Effective treatment modalities with early disease detection holds promise for further reducing the disease burden of ACR in orthotopic heart transplant patients, and STE, via the measurement of strain, may serve as a potential resource.…”

“…Improved surgical techniques and the development of new immunosuppressive drugs have significantly extended the survival of heart transplant (HTx) recipients. Acute rejection, which occurs in 25-32% of patients after HTx, still represents a mortality risk during the first 1-3 years following cardiac transplantation ( 1 ). The current standard for graft rejection surveillance is endomyocardial biopsy (EMB), an invasive procedure with rare but potentially serious complications ( 1 ).…”
