Acute Cervical Spontaneous Spinal Epidural Hematoma Presenting with Minimal Neurological Deficits: A Case Report

Abstract: IntroductionSpontaneous spinal epidural hematoma (SSEH) is an uncommon but potentially fatal condition. The increased bleeding tendency associated with anticoagulant medications has been proven to increase the risk of SSEH. The symptoms of SSEH usually begin with sudden severe neck or back pain and are followed by neurological deficits. However, some cases present with only axial pain or with radicular pain similar to herniated disc disorders.Case PresentationA 28-year-old healthy man developed a sudden onset … Show more

“…Spontaneous resolution of SEH usually occurs several hours to days after onset 23 . This has been hypothesized to be due to the spreading of the hematoma along the spinal epidural space though no conclusive evidence has been reached 24,25 .…”

“…10 Once the diagnosis of cervical SEH is made, surgical decompression such as early laminar decompression and hematoma evacuation is the current mainstay, 2,10,11 though some cases receiving conservative treatments also had favorable outcomes. 12 The prognosis of the disease depends on the preoperative neurological deficits and the time interval between the onset of symptoms and operation. 13 Thus, early recognition and thorough neurological examination are crucial and spontaneous SEH should always be taken into consideration in patients with ESRD and weakness.…”
