As a part of the final term, I was supposed to do a small research project. My colleague Marianne Tronrud and I decided to develop and validate a new, simple screening tool, which could be used in hospitals to detect malnutrition already at the patient's first contact with the nurse.To be able to validate the screening tool, which we named "The less complicated nutritional assessment (LCNA)", we needed to include patients that really had nutritional problems. Our supervisor, Ingvar Bosaeus, suggested using a pop ulation of patients with chronic obstructive pulmonary disease (COPD), since they had a high prevalence of malnutrition.During four weeks, Marianne and I performed the Subjective Global Assessment tool (which was the tool toward we validated LCNA) in 29 COPD patients admitted to Sahlgrenska University Hospital. This was my first encounter with COPD patients.After one year of military service and two years as a d ietitian in Norway, I got the possibility to work in a research project at the Department of Clinical Nutrition, Göteborg University. The aim of the project was to investigate the physical activity in Swedish adolescents. My interest in COPD patients had been triggered during my final year as undergraduate student, and contacts were therefore taken between the Department of Clinical Nutrition and the Depart ment of Respiratory Medicine and Allergology. We hypothesized that bronchodilator treatment not only improves hyperinflation and endurance capacity but also muscular efficiency in stable chronic obstructive pulmonary disease (COPD). We aimed to demonstrate that tiotropium and salmeterol improve muscular efficiency compared with placebo. Twenty-five COPD patients were studied, including 20 males of mean (standard deviation) age 62 years (7 years) with baseline forced expiratory volume in 1 second of 41% (10%) predicted, and maximal workload of 101 Watt (36 Watt). Subjects were randomized for 6-week treatment with tiotropium 18 μg once daily, salmeterol 50 μg twice daily, or placebo using a double-blind, crossover design. Muscular efficiency and endurance time were measured during cycling at 50% of maximal work load. Resting energy expenditure was measured using a ventilated hood. Muscular efficiency after tiotropium, salmeterol, and placebo treatment was 14.6%, 14.4%, and 14.4%, respectively (P > 0.05), and resting energy expenditure was 1485 kcal/24 hours, 1709 kcal/24 hours, and 1472 kcal/24 hours (P > 0.05), respectively. Endurance time after tiotropium treatment was significantly higher than that after placebo (27.0 minutes versus 19.3 minutes [P = 0.02]), whereas endurance time after salmeterol treatment was not higher than that after placebo (23.3 minutes [P = 0.22]). In this small study, we were not able to demonstrate that bronchodilator therapy improved muscular efficiency. Apparently, reduced costs of breathing relative to total energy expenditure were too small to be detected.

Inhaled beta 2 -adrenoceptor agonists (beta 2 -agonists) are among the most used substances in competitive sports. The 2020 Prohibited List issued by the World Anti-Doping Agency restricts use of all selective and non-selective beta 2 -agonists in-and out-of competition with few exemptions. Formoterol, salbutamol, and salmeterol are allowed by inhalation within defined dosing limits. These restrictions are in place because supratherapeutic use of beta 2 -agonist has the potential to be anabolic and to enhance performance, as well as due to potential side effects. Despite substantial documentation that beta 2 -agonists exert anabolic and lipolytic actions, these actions are not widely recognized. Furthermore, a common misconception is that the inhaled route does not exert these effects. However, given the high relative systemic bioavailability via the inhaled route, inhalation at high doses can also exert anabolic and lipolytic actions. In this review, we highlight the anabolic and lipolytic actions beta 2agonists can exert, regardless of the type of beta 2 -agonist and the route of administration. The doses needed to provide such effects are also associated with adverse effects and would in most cases be detected in routine doping control. Notwithstanding, the beta 2 -agonist regulations are associated with some challenges and given their ability to induce muscle growth and to enhance performance, it is important to continue developing effective detection strategies to prevent potential misuse of beta 2 -agonists while allowing treatment of asthmatic subjects without causing adverse side effects or ergogenic actions.
