Acute gastric volvulus and congenital diaphragmatic hernia, case report and review

Abstract: Congenital diaphragmatic hernia (CDH) is the result of the incomplete fusion and closure of the pleuroperitoneal canal during the fetal development. CDH is usually diagnosed prenatally but, if undiagnosed, the clinical presentation ranges from asymptomatic children to serious respiratory or gastrointestinal symptoms. Acute gastric volvulus associated with CDH is a rare surgical emergency in children. We report two cases of acute gastric volvulus associated with CDH and review the literature.

“…However, some patients with a CDH also present themselves with acute gastrointestinal problems and chronic dietary problems. 1,2 CDH has previously been associated with other congenital malformations. The best known is the occurrence of intestinal malrotation, which is relatively common in these neonates due to abnormal rotation of the embryological midgut.…”

“…Despite being most common in reproductive age women as a result of “acquired” ligamentous laxity, wandering spleen is also described in pediatric age with a peak of incidence in male under the first year [ 3 , 7 ]. In this group a “congenital” cause has been proposed, secondary to a failure in the development of splenic ligaments, from the dorsal mesentery, that anchor it to the stomach, diaphragm, colon, and retroperitoneum; as a result, the spleen is sustained only by its vascular pedicle with an increased risk of torsion [ 1 , 4 6 ]. The association with other congenital malformations as prune-belly syndrome, renal agenesis, congenital diaphragmatic hernia and eventration, and gastric volvulus has been previously described [ 4 , 5 , 12 ]; furthermore, a familiar form has been proposed [ 13 ].…”

“…Anterior gastropexy with or without phrenofundopexy and/or esophagocardiopexy, fundoplication, and gastrostomy are the surgical options proposed; in case of sever gastric ischemia and necrosis, a gastric resection should be necessary. Laparoscopic and laparotomic techniques are both described [ 2 , 6 , 19 , 23 ].…”

“…Среди провоцирующих факторов называют травмы и оперативные вмешательства на органах брюшной полости, приём чрезмерного количества пищи. Существуют предположения о связи гипер-трофического пилоростеноза с возникновением заворота желудка у детей грудного возраста [4].…”
