Acute hospital preoperative assessment redesign: streamlining the patient pathway and reducing on-the-day surgery cancellations

Abstract: IntroductionAvoidable surgery cancellations in an acute trust were often attributed to inadequate preoperative assessment. These assessments, undertaken shortly before surgery, were delivered across eight different locations, 60% by a central nursing team and the remainder by other healthcare professionals. There was inconsistency in what and who were assessed, and inadequate time to optimise patients. There was difficulty finding capacity for urgent patient assessment, plus a lack of a pool of ‘ready-assessed… Show more

“…Improving DOSA can enhance operational efficiency, reduce costs, and enhance patient experiences. Strategies include enhanced scheduling, thorough preoperative assessments, pre-surgery checklists, dedicated pre-admission areas, streamlined discharge processes, improved communication, utilization of technology, and continuous monitoring [ 27 , 28 ]. These initiatives optimize resource utilization, minimize waiting times, and ensure a smoother surgical pathway for patients.…”

“…Pre‐operative assessment of patients scheduled for major neurosurgical interventions often takes place in a tertiary centre distant from patients' homes. Utilising telemedicine and designing pathways that enable patients to see specialists and undergo pre‐operative investigations and optimisation locally can reduce patient travel and on‐the‐day cancellations [12]. Neurosurgery and neuro‐interventional radiology have a substantially negative impact on the environment in terms of energy usage, water and waste pollution [13, 14].…”
