Acute Kidney Injury from Excessive Potentiation of Calcium-channel Blocker via Synergistic CYP3A4 Inhibition by Clarithromycin Plus Voriconazole

Abstract: CYP3A4-inhibitors can potentiate the hypotensive effect of calcium-channel blockers. However, insufficient attention to such drug interactions may result in serious adverse reactions. A 71-year-old hypertensive man prescribed nifedipine was hospitalized for infectious endophthalmitis. Antimicrobial therapy with voriconazole lowered the blood pressure, and then clarithromycin further lowered it through the excessively elevated nifedipine concentration, leading to ischemic acute kidney injury. After the disconti… Show more

“…Voriconazole, a commonly used antifungal triazole, has potent inhibitory effects on CYP3A4 [ 5 ]. In the presence of CYP3A4-inhibitors, drugs that are metabolized by CYP3A4, such as nifedipine, will accumulate leading to toxicity [ 6 , 10 ]. Hypotension from co-prescribing voriconazole together with nifedipine has been reported [ 6 , 10 ].…”

“…This antibiotic is also a P-gp substrate and a strong competitive inhibitor. Therefore, its concentration and the resulting CYP3A4 inhibitory activity may be higher when it is simultaneously applied with lapatinib, which is also a P-gp inhibitor (18)(19)(20)(21)(22). It is very likely that clarithromycin will be applied to oncological patients treated with lapatinib.…”

“…One group (n = 6) received first clarithromycin and next lapatinib (I Lap+Clar ), second group (n = 6) received placebo (0.9% NaCl) and lapatinib (II Lap ). Clarithromycin was administrated at a dose of 25 mg per kg of body weight to the I Lap+Clar group (19,20). Lapatinib (100 mg/kg) was dissolved in 1 mL DMSO (dimethyl sulfoxide) and administered directly into the animal stomach using a gastric probe.…”
