Acute limb ischemia: contemporary approach

Abstract: Acute limb ischemia is a critical condition with high mortality and morbidity even after surgical or endovascular intervention. Early recognition is important, but a delayed presentation is not uncommon. Viability of the limb is assessed by motor and sensory function and with interrogating Doppler flow signals in pedal arteries and popliteal veins as categorized by Rutherford. Category IIa indicates mild-to-moderate threat to limb salvage over a time frame without revascularization. Limb ischemia is critical w… Show more

“…Due to the swelling of the musculature, arterial vessels are compressed and react to this additional mechanical irritation with a pronounced vasospasm. In such a situation, surgical pressure release via the incision of the fascia may relieve the tissue pressure and drain the edema [ 42 ]. In order to protect blood vessels from thrombosis, occlusions, and dangerous intraluminal pressure levels, a number of antithrombotic factors is expressed by the healthy endothelium.…”

“…Embolism recurs in 6-45 %. Therefore, anticoagulation is important to prevent recurrence [4]. Anticoagulation leads to a reduction in the propagation of the thrombus and accumulation of free-floating thrombus formations.…”

“…If no bypass is required, the arteriotomy should be closed using a vein patch to minimize potential stenosis [8]. Factors related to a successful outcome of the treatment of arterial embolism are an intact arterial intima, non-adherence of the thromboembolism to the intima, and a patent run-off vessel prior to embolization [4]. The catheter is introduced and pushed proximally beyond the thrombus and extracted after inflation of the balloon.…”

“…Hepatic active response is a predictable reaction, which belongs to the multiply organ failure syndrome. Association of IRI as the cause of such pathological effect in the organism was proved by a couple of researchers [32][33][34][35][36].…”
