Acute liver failure due to liver parenchymal infiltration with acute myelogenous leukaemia in a patient with myelodysplastic syndrome

Abstract: Liver involvement by acute leukaemia is rare and has a high mortality rate despite treatment. We report a case of a 66-year-old woman undergoing treatment for myelodysplastic syndrome with Vidaza (azacitidine) who presented with abnormal liver function tests. Despite negative serologic testing and unremarkable abdominal MRI, she continued to have significant elevation in bilirubin and international normalised ratio and worsening mental status. Liver biopsy was obtained and consistent with acute myelogenous leu… Show more

“…The first report of ALF due to AML was by Buchler and Cline [14] who described a young female with initially vaginal lymphoma; no details of the AML were provided in the case report. Seven subsequent cases of AML presenting as obstructive jaundice or ALF without distinguishable obstructive masses have been reported [12, 1520] (see Table 1). All patients had abnormal blood counts that were reflected as anaemia and/or thrombocytopenia and/or leukocytosis.…”

“… [6] Additionally, this pretreatment strategy demonstrates the potential value of cytoreduction as a temporizing measure for AML with hyperleukocytosis and extramedullary manifestations. Specifically, AML with hepatic infiltration appears to confer a worse outcome because its rapid progression limits the time available to delineate the etiology of obstructive transaminitis and initiate induction therapy before patient demise [3] . The concerns of initiating 7 + 3 induction therapy when obstructive jaundice is due to non-AML-related causes are two-fold.…”

“…In another case, chemotherapy was initiated in a patient with myelodysplastic syndrome who unknowingly had AML infiltration of the liver. This patient observed worsening liver status and faced rapid demise due to liver failure prior to initiation of definitive treatment [3] . These cases highlight the challenges associated with the diagnosis of AML with extramedullary manifestations as well as the perils with timing the initiation of chemotherapy.…”
