Neonatal osteomyelitis is said to be rare and this could be due to missed diagnosis following unusual presentation or as a result of underreporting. There is a paucity of reports from the subregion though its occurrence has been documented. Diagnosis is clinical with supporting laboratory and radiological investigations and treatment is with intravenous antibiotics. We report the case of a term neonate with osteomyelitis who was admitted on the fourth day of life on account of bilateral non-traumatic leg swelling. A high index of suspicion coupled with thorough clinical examination and investigation is needed to ensure that such unusual presentations are not missed.
