Acute renal failure related to oxaliplatin-induced intravascular hemolysis

Abstract: Oxaliplatin is an effective chemotherapeutic agent frequently used in the treatment of colorectal carcinoma. Rare cases of renal failure and hemolytic reactions have been reported as separate side effects of oxaliplatin. Here we present a clinical picture of immune-related intravascular hemolysis and acute tubular necrosis in a patient receiving this drug. This case suggests a mechanistic explanation of renal failure in patients treated with oxaliplatin.

“…Excluding the present case, seven cases of AKI have been reported after treatment with oxaliplatin (Table 4) since 1999, when the first case of hemolytic anemia with AKI was reported by Desrame et al [6][7][8][9][10][11][12]. All cases were accompanied by anemia and four cases were identified as immune-related hemolytic anemia with positive results for DAT [6,8,11,12].…”

“…All cases were accompanied by anemia and four cases were identified as immune-related hemolytic anemia with positive results for DAT [6,8,11,12]. In contrast, acute tubular necrosis was considered to be caused by direct renal toxicity from oxaliplatin in two cases [7,9] and the remaining case was reported as hemolytic uremic syndrome with negative DAT results [10].…”
