Thyroid abscess is a rare encounter due to its unique histological and structural features. It is usually associated with some form of congenital anomalies in pediatrics especially if recurred. Early recognition and treatment are of paramount importance in preventing complications. Atypical presentation can occur if the patient is improperly treated prior to presentation. Conservative management is becoming the mainstay of treatment unless there is a risk of airway compromise or extension. We report the case of a 15-month-old female who presented with anterior neck swelling. She received oral antibiotics prior to her visit and did not exhibit severe systemic illness despite her disease extension. She was found to have a thyroid abscess originating from the left thyroid lobe extending to the mediastinum. No congenital anomalies were found. She was managed with open drainage and her cultures grew Streptococcus pyogenes.

Acute suppurative thyroiditis is a rare disease because the thyroid gland is remarkably resistant to infection. We present a 2-year-old girl with refractory acute suppurative thyroiditis due to a pyriform sinus fistula (PSF). She complained of fever and painful anterior neck swelling. Her condition did not completely improved by multiple parenteral antibiotics along with incision and drainage. Barium esophagogram to detect PSF demonstrated no specific finding. Computed tomography scan showed air bubble superior to the left thyroid gland which indicated a possible fistula connected to the pyriform sinus. An intraoperative laryngoscopy revealed a 2-mm-sized fistula opening. The fistula was successfully treated by chemocauterization with trichloroacetic acid.

Clinical presentations of pyriform sinus fistulas vary, and this sometimes leads to a delay in diagnosis and treatment. Recently, we experienced a case of recurrent cervical abscesses occurring after thyroidectomy in an adult affected by a bifidus pyriform sinus fistula. The diagnostic dilemma was related to the timing of events, with a single episode of acute suppurative thyroiditis having occurred 16 years before the onset of the more recent clinical scenario. An endoscopic approach allowed effective management of this clinical case.
